Implementation packet for AMDA clinical practice guidelines. pressure ulcers and pressure ulcer therapy companion: creating the tool Scott M. Bolhack, MD, CMD, FACP, TLC HealthCare Companies; Michael Smith, MD, Second Moment Consulting; Shirley Grant, LPN, TLC HealthCare Companies, Tucson, AZ Problem. Clinical Practice Guidelines (CPGs) developed by the American Medical Directors Association (AMDA) are not widely utilized. Method. Using the AMDA CPGs Pressure Ulcers and Pressure Ulcer Therapy Companion, we created an Implementation Packet (IP) including: a step-diagram based upon the algorithms within each CPG; a checklist-based assessment tool utilizing and referencing CPG information ; each CPG; and supplemental appropriate additions. The Pressure Ulcer IP and a simple evaluation survey were presented to nursing management in the facilities where the primary author is medical director. Results. Four facilities responded. Most indicated they would not use the packets because they were too long or too complicated. One facility is restricted to using corporate forms. Favorable comments were that the assessment tool would help direct care and would be useful for residents with pressure ulcers. Estimated time to complete the assessment tool was 30 to 45 minutes each. Conclusion. The current AMDA Pressure Ulcer CPGs offer an excellent guide, but utilization of their algorithms is inconvenient. We converted the CPGs into a working documentation tool. Our first presentation of the tool, with references, indicated that its usage would be limited. Attempts continue to simplify the IP without limiting its educational aspects.
