Management and rapid healing of a traumatic BKA in a forty-four year old motorcyclist using a hydrocellular polyurethane foam dressing Alan J. Cantor, DPM, Burn/Wound & Hyperbaric Medicine Center, Nassau University Medical Center, East Meadow, NY A 44-year old male arrived via helicopter following a serious motorcycle accident resulting in a traumatic amputation of the left foot at the ankle level. Debridement of nonviable bone and soft tissue was performed. He developed a severe bone infection, requiring more debridement, followed by wet-to-dry dressings. The patient was referred to our wound center because of a non-healing stump wound. The treatment program was changed and consisted of culture derived antibiotics, sharp and enzymatic debridement, and application of a hydrocellular polyurethane foam* to manage the exudate and cushion the stump. This program was continued for two weeks. The stump wound completely epithelialized. The patient was fitted with a prosthesis and is successfully ambulating. The addition of a hydrocellular polyurethane foam dressing controlled exudate, provided necessary cushioning to the bony stump prominence and reduced costs by facilitating rapid wound closure. Appropriate dressing selection based upon proper patient assessment enabled this patient to quickly heal, begin a rehabilitation program and to return home to resume normal activities. * Allevyn Non-Adhesive dressing, Smith & Nephew Inc, Largo, FL |
