Healing a one year old full thickness heel pressure ulcer in an elderly diabetic: rapid debridement, wound bed preparation and oasis with V.A.C. therapy Alan J. Cantor, DPM, Donna Hangan, NP, Burn/Wound & Hyperbaric Medicine Center, Nassau University Medical Center, East Meadow, NY This wheel chair bound, 80-year old female is an insulin-dependent diabetic, with a history of peripheral vascular disease (ABI = 0.48). She had severe bilateral distal arterial insufficiency, pacemaker implant, and renal impairment. An initial skin graft failed and she was referred to the wound and hyperbaric center for treatment by the vascular surgery department. After a year of treatment in which the wound did not heal, she was admitted into the hospital where immediate surgical debridement of the ulcer and antibiotic therapy took place in the OR; she underwent Vacuum Assisted Closure (VAC) and the wound was further cleansed with Accuzyme ointment. The wound was debrided once more over the next two weeks and she was then transferred to a nursing home; VAC continued for one more week. Once free of necrosis, the wound was treated with Oasis® Wound Matrix in conjunction with vacuum Assisted Closure (VAC). A second Oasis matrix was applied three weeks later, covered with alginate dressing. A total of five Oasis dressings were applied over a 10 week period, at which time the ulcer had completely healed.. |
