Polymeric membrane dressings for the use on a traumatic left knee wound Laura Barnes, RN, MSN, WOCN, CWCN, APN-CNS Clinical Problem: An 82-year-old man presents with a left knee wound caused by a fall. Patient has a history of hypertension, venous insufficiency and a left knee replacement. Patient developed cellulites and 100% necrosis to the left knee wound bed. The wound measured 7cm x 5cm with a depth 1cm and erythema present to surrounding skin greater than 3cm, odor present and patient complained of wound pain. A WOC nurse was consulted in the Home Health Care Setting and recommendations included hospitalization for sharp debridement, albumin and CBC labs, rule out osteomylitis and include possible antibiotic therapy. During hospitalization wound care management included wet to dry dressings. Patient was discharged home after 8 weeks of hospitalization. .Current Clinical Approach: A WOC Nurse was reconsulted for wound care management. The left knee wound measured 4cm x 3cm depth .5cm with 100% granulation tissue noted and moderate amount of serous drainage. Plan of wound care management included to apply polymeric membrane dressings daily and wrap with kerlix /ace wrap. .Patient Outcome: The patient reported relief of wound pain. With the use of polymeric membrane dressings the left knee wound healed within 9 weeks. .Conclusion: Polymeric membrane dressings are excellent dressings to use on managing exudate, maintaining a moist wound bed and relieving wound pain and during dressing changes. .Behavioral Objectives:.1) Discuss problematic issues related to traumatic left knee wound..2) Identify polymeric membrane dressings used to manage exudate and providing a moist wound healing..3) Discuss polymeric membrane dressings to manage and releive wound pain.
