Achieving improved outcomes through coordination of interdisciplinary care team and advanced wound technology Kathleen Judge, MSN, APRN,BC,CWOCN, Christopher LaRosa, DPM, Virtua Health, Berlin, NJ A 50-year-old man presented to ED with full thickness plantar foot wound infection and cellulitis. Patient had no insurance and was found to have undiagnosed diabetes mellitis. Patient required extensive collaboration between a large interdisciplinary care team and partnership with multiple Wound Company Vendors as inpatient and outpatient to coordinate his physical and social care needs. It was discovered that through a coordinated and comprehensive plan of care plus utilizing multiple advanced wound care treatments and technologies such as vacuum assisted closure therapy and extracellular matrix grafts that successful wound healing was accomplished. From this experience, other wound clinicians can learn how to individualize a challenging patient's plan of care to incorporate wound treatment needs, reimbursement and cost containment issues, patient education and coordination of care for improved outcomes.
