Case Study

A fresh look at common therapy for full-thickness wounds

Yael Steinfeld, RNP, MS, Rhonda Burno, NP, Israel Coutin, MD

This study compares the use of the traditional enzymatic treatment for a full thickness pressure ulcer, against a combination of zinc oxide and anti-fungal ointment

The patient presented to our LTC facility with full thickness wounds on both buttocks. We applied low-air-loss mattress and started daily dressing with enzymatic ointment (Accuzyme). The response to enzymatic treatment was minimal.. .The clinical picture remained of a wound with 100% yellow slough and errhythema in surrounding tissues on both buttocks

We applied our combination of Zinc Oxide and Anti-Fungal to the patient's right buttock full-thickness wound, and the enzymatic ointment with anti-Fungal to the left buttock

Within 1 week the the right buttock wound resolved, while the left buttock wound remain nearly the same

This application had been successful in many other cases of full thickness wounds and pressure ulcers.
