A study of combined interventions: negative pressure wound therapy and a 100% sodium carboxymethylcellulose fiber dressing with Ionic silver for the M

This case study is an example of using a combination of Negative Pressure Wound Therapy (NPWT)1 with a 100% Sodium Carboxymethylcellulose fiber dressing with ionic silver (Na CMC)2. A 64 year old femaile patient with a surgical flap over a nonhealing Stage IV pressure ulcer over her left isheal process was initiated on this treatment regimen. Initial wound measurements were 15 cm x 9cm x 8cm (L x W x D). The wound was cleansed with normal saline and the moistened Na CMC dressing was placed as the contact layer, the suction set and pump was then placed per standard procedure. Dressing changes were twice weekly. The Na CMC dressing was chosen due to the wound location and risk of infection and contamination to the wound. Additionally, the patient was placed on a group 3 duel therapy mattress ; had a foley catheter in place for urine diversion ; and was montiored by nutritional services weekly. In 10 weeks, the wound size decreased by 96% and the wound depth decreased by 90%. NPWT was discontinued at that time and topical treatment continued with Na CMC dressings every 3-5 days. Wound closure was acheived within the following two weeks.
